I have 3 stories lol
1.. working on a minesite at Port Hedland WA and our "peggy" filled up the fuel cart with petrol (1000L) and then went around and filled up all the earthmoving equipment, etc.
2.. I was at a servo when a guy walks in and says the diesel on pump 4... the attendant says "dont you mean the 400L of petrol?" the guy just filled his concrete pump..
3.. And then there was me, who filled up a brand new $12000 3-phase generator with petrol.

Luckily enough the worst damage in all of these was the loss of a peggy

Good stories mate, i like them. We get lots at my work but just cars,4wd, smaller scale. I do know of a $170G Merc that was filled with petrol first fill in its life. Insurance wrote it off. My daughter filled her Pathfinder, then drove 20ks to my place and asked if that was ok to put ULP in her Diesel. I drained,changed filters and still ok 20th Ks later. Fingers crossed for future.
